I requested it from bisty_icons here at LJ. In order to get the personalized icons, you have to join the community and keep an eye out for when she opens up the offers, as they're usually not open very long. In fact, I have my mail set so that I'm notified when she posts, so I don't miss out, having learned by sad experience only too many times. ;)
I have a whole lot of different icons, both with the "Coastal Spirit" name, and with "Gale".
